Observation

A child was found by her father on the outside play area unattended and unsupervised. During the complaint assessment, the center director and a staff member reported that the child was left unsupervised for 5-7 minutes. Two staff members acknowledged that name to face head counts were not completed during the transition from the playground to the classroom and one staff member could not recall how many children were counted when the class transitioned from the playground to the classroom.
